If you received a medical record request from ICE, our Provider Help Desk is standing by to assist.  When you call, please have the ICE request packet documents available.  Our Help Desk attendant will ask for your provider number (CCN or UPIN).  Your provider number can be found on the medical record tracking coversheet.  You will also be asked to provide information about the specific medical record such as the ICE ID, study ID, patient name, etc.  This information can also be found on the coversheet.

You may email us at info@ice-llc.net.  Because of our strict adherence to laws governing the use and protection of patient privacy and security, please do not include any PHI/PII in your email.  Do include any non-patient specific questions that you have, along with your name, provider number (CCN or UPIN), and a phone number where we may reach you.

You may also send us a fax at 717-747-1040, but please abide by the security measures described in the paragraph above.

The work that our research partners do is very important and depends on timely participation of providers such as you.  Our staff is here to facilitate submission of the medical records needed for the study.  Please reach out to us by any of the means above.
